Using Ultra graphics levels means you're adding 4xMSAA and several very demanding rendering effects. Especially the ambient occlusions (can be set to full or half quality) really pull some teeth, lol. Now, Serious Sam 3 is a very old game from 2011, but devs made sure that the engine packed so many features that the game would require much gpu power for many years to come.
